Hi! I have kind of the same trouble.

I was working with the article ¨contact us¨ and then... BAM!!! the whole frontend is white!!!
I already did what you suggested (helper.php "modules/mod_qlform/" folder line 11...) and I notice that 'modQlformFiler' don´t match with the names of the file "modules/mod_qlform/classes/" I try two things: I erase the 'modQlformFiler' and I also leave it on line 11 helper.php file.

In both case this error appears
Fatal error: Class 'modelModqlform' not found

Can you help me?
